- Trinity Code of Conduct Form required
- Trinity Confidentiality of Patient & Proprietary Information and Information System Security Agreement required
- Trinity Specialty Specific Competency Testing must be completed within a year
- COVID-19 Vaccine proof required (Sept 2025 booster, declination or exemption with agency letterhead)
- MMR vaccination proof required (2 vaccinations or IgG titer)
- Hepatitis B vaccination series required (1st vaccination prior to start)
- Influenza Vaccine optional
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required
- Varicella vaccination proof required (1 vaccination or IgG titer)

- Trinity EPLS report required within past 30 days
- Trinity GSA/OIG report required within past 30 days
- Trinity Integrity and Audit Services Form signed prior to start
- Trinity Joint Commission Testing (CORE Exam within a year) required
- Trinity NSO report required within 30 days of start
- Trinity System Access Form must be completed and uploaded one week before start
- Trinity Skills Checklist required for clinical contractors
