The Med Care Manager/QMAP administers, documents and manages all prescribed medications per physician orders under the direction of the Wellness Director and in compliance with state regulations. The role ensures medication availability, accurate documentation, and patient safety while collaborating with nursing staff and residents.
Responsibilities- Administer and document medications following the five rights and physician orders.
- Order medications from pharmacy 7 days before running out and ensure timely delivery.
- Immediately document administered medications in the resident’s medical record with date, time and signature.
- Observe and report medication reactions or side effects to the licensed nurse.
- Take and record vital signs before administering medications that may affect vitals.
- Report medication errors to the Wellness Director without delay.
- Assist in developing and reviewing residents’ care plans with interdisciplinary teams.
- Maintain residents’ privacy, dignity and respect.
- Identify and correct unsafe conditions that could harm residents.
- Ensure physical comfort, safety and mental well‑being of residents.
- Assist residents with daily care tasks (toileting, grooming, bathing, feeding, dressing, monitoring intake, moving residents, responding to calls).

Education/
Experience:
Minimum high school diploma or equivalent; preferred previous healthcare experience, especially in Assisted Living or Memory Care, with medication administration experience. Proficient decision‑making centered on resident interest.
Certification: State approved Medication Administration training, First Aid certification as required by ALF regulations; QMAP in the state of CO (if applicable).
Continuing
Education:
Mandatory monthly in‑service education and training as required by law.
Other: Ability to read/write English, understand written and oral directions, communicate effectively in English, and exhibit good judgment in recognizing resident condition changes.
Industry
Experience:
Minimum three years in senior living; passion to work with seniors; prior Assisted Living community experience.
