Job Description

Licensed Practical Nurse (LPN) providing direct nursing care to residents and supervising the day‑to‑day activities of LPNs and nursing assistants. Responsible for adhering to federal, state, and local regulations, facility policies, and ensuring high‑quality staff and resident care.

Required Qualifications
  • Prior experience in long‑term care preferred but not required
  • Nursing degree from an accredited college or university
  • Current unrestricted license as a Licensed Practical Nurse (LPN) in the practicing state
  • Current CPR certification
Major Duties and Responsibilities
  • Direct daily activities of LPNs and certified nursing assistants in accordance with regulations and facility policies
  • Follow the nursing model to promote holistic care for residents
  • Ensure compliance with policies and procedures by staff
  • Participate in admission, discharge, and transfer processes
  • Assess changes in resident status, notify physicians and families, and document accordingly
  • Transcribe physician orders into the medical record and carry out orders as written
  • Prepare and administer medications per physician orders and observe for adverse effects
  • Perform wound treatments, observe changes, and document as required
  • Schedule follow‑up appointments and transportation needs
  • Provide nursing leadership to assigned personnel
  • Deliver direct care skills, including colostomy changes, tube feedings, wound care, suctioning, IV administration, etc.
  • Conduct rounds to ensure resident needs are met and staff duties are performed
  • Collaborate with interdisciplinary team members to meet residents’ holistic needs
  • Participate in management team meetings to discuss census, resident status, and concerns
  • Assist in evaluating nursing personnel performance and implement discipline as directed
  • Report concerns, grievances, incidents, or unusual occurrences to appropriate authorities and participate in investigations
  • Participate in end‑of‑life care as required
  • Maintain adequate stock of medications, supplies, and equipment; notify needs promptly
  • Initiate, review, and update care plans as needed
  • Promote teamwork, mutual respect, and effective communication
  • Participate in inspection survey processes and QAPI activities when assigned
  • Ensure infection control principles are followed and assist in correcting deficiencies
  • Perform other duties as assigned by the unit manager or director of nursing

  • Treat all residents with dignity and respect; promote and protect residents’ rights
  • Maintain compliance by adhering to all facility policies, state/federal regulations, and business standards
  • Follow safety and hygiene measures to protect residents and staff
  • Maintain confidentiality of protected health information
  • Report non‑compliance, retaliation, discrimination, abuse, neglect, or hazards to appropriate personnel
  • Participate in life safety and emergency drills and fulfill responsibilities during emergency activation
  • Report work‑related injuries and illnesses immediately
  • Complete all assigned training and competency evaluations as required by employment conditions

  • May work beyond normal hours, including weekends and holidays as necessary
  • Subject to frequent interruptions and possible exposure to infectious materials and hazardous chemicals
