Licensed Practical Nurse (LPN)
Licensed Practical Nurse (LPN) provides skilled nursing care to home health and hospice patients, delivering compassionate, patient-centered care that maintains comfort, dignity, and quality of life in patients' homes.
Key Responsibilities:- Provide skilled nursing care to home health and hospice patients per physician orders and individualized care plans.
- Monitor and document patient conditions, symptoms, and responses to treatment.
- Administer prescribed medications and treatments safely and accurately.
- Assist patients with disease management, symptom control, and comfort measures.
- Educate patients and family members on medications, treatments, and ongoing care needs.
- Collaborate with physicians, registered nurses, therapists, social workers, and other interdisciplinary team members.
- Observe and report changes in patient status promptly.
- Maintain accurate and timely electronic medical documentation.
- Promote patient safety while ensuring compliance with state and federal regulations.
- Participate in care conferences and contribute to individualized patient care planning.
- Deliver compassionate emotional support to patients and their families throughout the continuum of care.
- Current Licensed Practical Nurse (LPN) license in Idaho or eligibility for Idaho licensure.
- Graduate of an accredited Practical Nursing program.
- Minimum of 1 year of Home Health and Hospice nursing experience.
- Current Basic Life Support (BLS) certification.
- Valid driver's license with reliable transportation and current auto insurance.
- Ability to travel throughout assigned service areas.
- Strong assessment, communication, and organizational skills.

- Excellent time management and independent decision-making skills.
- Compassionate bedside manner with strong interpersonal communication.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Knowledge of home health regulations and hospice philosophy of care.
