Join our dynamic team at our offsite Primary Care Clinic | Senior Living, Autonomy and Meaningful Relationships

Schedule

0.8 FTE (64 hours per pay period), Monday through Friday. Schedule is subject to change based on patient and clinic needs.

Responsibilities
  • Partner with a Nurse Practitioner to deliver comprehensive primary care services, including preventive care, chronic condition management, follow-up visits, and care coordination.
  • Provide direct patient care within the LPN scope of practice, including collecting health information, performing clinical procedures, documenting care, and supporting medication reconciliation and care plan follow-up.
  • Support clinic operations and administrative functions, including patient scheduling, referral coordination, documentation, phone calls, and other activities that contribute to efficient clinic workflows (approximately 20% of role responsibilities).
  • Build trusted relationships with residents, families, community staff, and care team members while providing education and support to help patients navigate their healthcare journey.
  • Help develop and refine clinic workflows, processes, and operational standards while supporting implementation of this growing care model.
Qualifications Experience
  • Prior LPN experience preferred but not required.
  • New graduates are encouraged to apply. Candidates should have previous healthcare experience in a clinical setting, such as Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), or a similar direct patient care role.
Education
  • Graduation from a Licensed Practical Nursing Program
Certifications
  • Current BLS-CPR Certification from the American Safety and Health Institute, American Heart Association or American Red Cross
  • Current Licensed Practical Nuse (LPN) licensure in the State of practice
