Licensed Practical Nurse
Licensed Practical Nurse providing home health and hospice care in Boise, Idaho. Deliver compassionate, one-on-one patient care to maintain comfort, dignity, and quality of life in patients' homes.
Key Responsibilities:- Provide skilled nursing care per physician orders and individualized care plans
- Monitor and document patient conditions, symptoms, and responses to treatment
- Administer prescribed medications and treatments safely and accurately
- Assist patients with disease management, symptom control, and comfort measures
- Educate patients and family members on medications, treatments, and ongoing care needs
- Collaborate with physicians, registered nurses, therapists, social workers, and other interdisciplinary team members
- Observe and report changes in patient status promptly
- Maintain accurate and timely electronic medical documentation
- Promote patient safety while maintaining compliance with state and federal regulations
- Participate in care conferences and contribute to individualized patient care planning
- Deliver compassionate emotional support to patients and their families throughout the continuum of care
- Current Licensed Practical Nurse license in Idaho or eligible for Idaho licensure
- Graduate of an accredited Practical Nursing program
- Minimum of 1 year of Home Health and Hospice nursing experience
- Current Basic Life Support (BLS) certification
- Valid driver's license with reliable transportation and current auto insurance
- Ability to travel throughout assigned service areas
- Strong assessment, communication, and organizational skills
Skills:
- Excellent time management and independent decision-making skills
- Compassionate bedside manner with strong interpersonal communication
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Knowledge of home health regulations and hospice philosophy of care
- Commitment to delivering exceptional patient-centered care
- Experience with electronic medical records
