Position: Full Time LPN - Home Health & Hospice - Up to $80,000/Yr!

Licensed Practical Nurse

Licensed Practical Nurse providing home health and hospice care in Boise, Idaho. Deliver compassionate, one-on-one patient care to maintain comfort, dignity, and quality of life in patients' homes.

Key Responsibilities:
  • Provide skilled nursing care per physician orders and individualized care plans
  • Monitor and document patient conditions, symptoms, and responses to treatment
  • Administer prescribed medications and treatments safely and accurately
  • Assist patients with disease management, symptom control, and comfort measures
  • Educate patients and family members on medications, treatments, and ongoing care needs
  • Collaborate with physicians, registered nurses, therapists, social workers, and other interdisciplinary team members
  • Observe and report changes in patient status promptly
  • Maintain accurate and timely electronic medical documentation
  • Promote patient safety while maintaining compliance with state and federal regulations
  • Participate in care conferences and contribute to individualized patient care planning
  • Deliver compassionate emotional support to patients and their families throughout the continuum of care
Qualifications:
  • Current Licensed Practical Nurse license in Idaho or eligible for Idaho licensure
  • Graduate of an accredited Practical Nursing program
  • Minimum of 1 year of Home Health and Hospice nursing experience
  • Current Basic Life Support (BLS) certification
  • Valid driver's license with reliable transportation and current auto insurance
  • Ability to travel throughout assigned service areas
  • Strong assessment, communication, and organizational skills
Preferred

  • Excellent time management and independent decision-making skills
  • Compassionate bedside manner with strong interpersonal communication
  • Ability to work independently while collaborating effectively with an interdisciplinary team
  • Knowledge of home health regulations and hospice philosophy of care
  • Commitment to delivering exceptional patient-centered care
  • Experience with electronic medical records
