This full-time Hospice RN Case Manager role in Boise, Idaho provides compassionate, skilled nursing care to patients in their homes while coordinating interdisciplinary hospice teams. The position requires an Idaho RN license, nursing degree, and BLS/CPR certification, with experience in hospice or palliative care preferred. The successful candidate will manage patient care plans, educate families, and maintain rigorous documentation and regulatory compliance.
Key Responsibilities:Provide compassionate, skilled nursing care to hospice patients in homes and other settings.
Serve as primary case manager for assigned patient caseload.
Conduct comprehensive nursing assessments of physical, emotional, psychosocial, and spiritual needs.
Develop, implement, and regularly update individualized hospice plans of care.
Coordinate care with physicians, aides, social workers, chaplains, volunteers, and other team members.
Monitor patient condition changes and adjust plans accordingly; communicate significant changes to physicians and medical director.
Educate and provide emotional support to patients and families on disease progression, symptom management, medications, and end-of-life care.
Manage pain and other symptoms, administer medications per orders, and evaluate intervention effectiveness.
Assist families in understanding hospice services and comfort-focused care goals.
Coordinate care transitions and maintain accurate, timely clinical documentation.
Participate in interdisciplinary meetings, collaborate with team to ensure continuity and quality of care.
Respond to urgent patient and family needs within hospice scope; maintain confidentiality and adhere to regulations.
Current and unrestricted Registered Nurse RN license in Idaho or eligibility for Idaho RN licensure.
Associate Degree in Nursing (ADN) from accredited program;
Bachelor of Science in Nursing (BSN) preferred.Current BLS/CPR certification for healthcare providers.
Valid driver's license and reliable transportation; ability to meet automobile insurance requirements.
Strong clinical assessment, documentation, communication, and organizational skills.
Skills:
Previous hospice, palliative care, home health, or related nursing experience.
Previous experience as a Hospice RN Case Manager.
Hospice or palliative care nursing experience.
Home health nursing experience.
Familiarity with Medicare hospice regulations and requirements.
Knowledge of pain management and symptom control.
Experience working with interdisciplinary healthcare teams.
Strong understanding of end-of-life care and family support.
Ability to work independently while maintaining effective communication with the hospice team.
Compassionate, patient-centered approach to care.
Strong time-management and case-management skills.
You hold an unrestricted Idaho RN license. You have hospice or palliative care nursing experience. You can manage caseloads and coordinate interdisciplinary teams
