Home Health​/Hospice Patient Care Coordinator​/Liaison Marketer

This role coordinates home health and hospice care by building referral relationships and connecting patients with appropriate services across the Boise area. The coordinator works directly with physicians, hospitals, and community partners to promote services and manage referrals from initial contact to admission.

• Develop and maintain relationships with physicians, hospitals, case managers, discharge planners, SNFs, ALFs, and other referral sources.

• Visit hospitals, healthcare facilities, physician offices, and community organizations throughout the Boise area.

• Promote Home Health and Hospice services and develop new referral opportunities.

• Manage and track referrals from initial contact through admission.

• Communicate with internal and external teams regarding referral status and admission needs.

• Assist with admissions, transfers, discharges, and insurance authorization processes.

• Maintain knowledge of Medicare, Medicaid, private insurance, and other healthcare payors.

• Participate in networking, community outreach, and healthcare marketing activities.

• Maintain accurate referral, census, and patient information.

• Provide exceptional customer service to patients, families, and healthcare partners.

You have strong relationships with physicians and referral sources. Experienced in managing referrals from contact through admission. Skilled at community outreach, marketing, and patient liaison duties
