The Home Health Director of Nursing / Clinical Manager is a full‑time, on‑site role based in Bolingbrook, IL. This role oversees all clinical operations, including the development, implementation, and evaluation of patient care plans to ensure high‑quality, compliant home health services. Daily responsibilities include supervising nursing staff, coordinating interdisciplinary care, reviewing clinical documentation, and guiding clinicians in best practices and regulatory standards.
The Director of Nursing / Clinical Manager collaborates with leadership to improve processes, supports staff training and performance evaluations, and serves as a key clinical resource for clients, families, and care teams. This position also participates in quality assurance activities, addresses clinical concerns, and helps maintain a culture of safety, compassion, and professionalism.
Qualifications- Active Registered Nurse (RN) license in Illinois and prior experience in home health or related clinical settings.
- Demonstrated leadership abilities in supervising nursing teams, managing clinical operations, and coordinating interdisciplinary care.
- Strong skills in care planning, clinical assessment, documentation, and adherence to state, federal, and accreditation regulations.
- Effective communication, coaching, and relationship‑building skills with clients, families, and healthcare professionals.
- Proficiency in using electronic health records, clinical software, and basic office technologies to support efficient care delivery.
- Ability to prioritize multiple tasks, make sound clinical decisions, and maintain high standards of quality and safety.
- Bachelor’s degree in nursing (BSN) preferred; prior management or supervisory experience in homecare or community health is highly beneficial.
