- Conduct comprehensive patient assessments and document symptoms, progress, and outcomes accurately.
- Perform initial home health evaluations and ongoing reassessments as needed.
- Develop and implement individualized plans of care.
- Complete OASIS documentation accurately and in a timely manner.
- Supervise Home Health Aides (HHAs) and provide clinical guidance as appropriate.
- Ensure compliance with Medicare regulations and agency policies.
- Communicate changes in patient condition to physicians and other healthcare providers.
- Coordinate care with interdisciplinary team members to support optimal patient outcomes.
- Complete all required documentation and paperwork efficiently and on schedule.
- Educate patients and caregivers regarding disease processes, medications, treatments, and care plans.
- Deliver high-quality nursing care in accordance with professional standards and agency guidelines.
- Provide skilled nursing services, including wound care, IV therapy, line maintenance, and ongoing assessments.
- Collaborate effectively with healthcare professionals, patients, and family members to ensure continuity of care.
- Current and unrestricted Illinois Registered Nurse (RN) license.
- Home health experience preferred.
- Proficiency with OASIS documentation.
- Knowledge of Medicare regulations and standards.
- Excellent clinical assessment, communication, and organizational skills.
- Ability to work independently while contributing to a collaborative team environment.
- Valid driver's license and reliable transportation.
