Citizens Memorial Hospital (CMH) is hiring a Social Worker to care for our patients at the Dialysis Clinic in Bolivar, Missouri.

CMH is made up of 34 primary and specialty clinics, 6 long-term care facilities, and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

About the area:Located in the heart of the Ozarks, our main campus is in Bolivar, Missouri,where you'll find ahometown feel and affordable housing options. About the department: Supporting patients of all ages, the Social Services Department is available 24/7 to offer assistance with discharge planning, resource referrals, and consultative services. Working as part of an interdisciplinary team, Social Workers and Designees provide clear communication to patients, family members, and other team members.

A typical day includes: Providing support and services to patients families and caregiversatthe Dialysis Clinic.

  • Develops assessments when indicated, which examine the current problem(s) as well as relevant biopsychosocial and environmental factors.
  • Assesses characteristics of patient's support systems, socio-economic needs, and spiritual/cultural needs for developing action plans for additional support, as applicable.
  • Performs care management functions including referrals to appropriate healthcare, social agencies, and community-based organizations.
  • Provides information and referrals to patients and their families regarding the biopsychosocial aspects of patients' adaptation to diagnosis, treatment, and recovery expectations.

This position is: PRN working 8 hours each week between the hours of 8:00am - 5:00pm, Monday - Friday

Job Requirements:

  • Education:

    Master's Degree in Social Work from an accredited institution
  • Experience:

    1 year of experience preferred
  • Skills:

    Patient care, customer service, problem-solving, listening
  • Licensure/Certification: MSW is required
  • Physical:
    Ability to sit for prolonged periods of time.
