Registered Nurse - Clinic

Shift – Full Time – Days – 8:00 am – 5:00 pm

Citizens Memorial Hospital (CMH) is recruiting a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Medical Assistant (MA) to care for our patients in the Pleasant Hope Family Medical Center. CMH is made up of 34 primary and specialty clinics, 6 Long‑Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk‑in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered.

This rural health clinic (RHC) provides complete family care, well‑baby exams, well‑woman exams, Medicare wellness visits, behavioral health visits, immunizations and same‑day appointments. The clinic is recognized by the Compliance Team as an Exemplary Provider. Exemplary Provider status is granted to those healthcare providers who demonstrate outstanding patient care practices and compliance to the Compliance Team’s comprehensive Safety‑Honesty‑Caring quality standards.

The RN, LPN or MA is an integral part of the healthcare team, working with the physician and the nurse practitioner. The role is responsible for: intake of patient reason for visit, medication reconciliation, verification of the patient’s medical/family/social/surgical history, assessment of patient utilizing required questionnaires, assisting providers with in‑office procedures, providing education for patient and/or caregiver, providing phone triage, and promoting preventative measures to optimize patient outcome.
