What You'll Do
As a MDS Corodinator, you will conduct and coordinate the development and completion of the resident assessment in accordance with the federal and state requirements and the policies and goals of this facility.
Responsibilities include:
- Coordinate the timely and accurate completion of all resident assessments (MDS) in accordance with federal and state regulations.
- Develop and update individualized care plans based on assessment outcomes, ensuring measurable goals and defined responsibilities for each service.
- Ensure initial MDS assessments are completed within 14 days of admission and comprehensive care plans within 21 days.
- Complete quarterly, annual, and significant change assessments and reviews in accordance with regulatory timelines.
- Collaborate with the Interdisciplinary Care Plan Team to create comprehensive assessments and care plans for each resident.
- Involve residents, families, and legal representatives in the assessment and planning process when appropriate.
- Maintain effective, cooperative relationships with healthcare professionals, consultants, and regulatory agencies involved in resident care.
- Evaluate residents' conditions and medical data to determine the need for additional or revised assessment activities.
- Educate and ensure compliance among assessment team members regarding accuracy, completeness, and legal certification responsibilities.
- Participate in regulatory surveys and assist in preparing for inspections related to MDS and care planning functions.
- Conduct or support in-service training for staff related to assessment procedures, MDS documentation, and care planning practices.
- Ensure timely electronic transmission of MDS assessments to appropriate state and federal agencies.
Qualifications
- Nursing degree from an accredited institution with an active, unencumbered RN license in this state.
- Minimum of one year of experience in a hospital, long-term care, or related healthcare setting.
- Strong knowledge of nursing practices, long-term care regulations, and resident care standards.
- Demonstrated leadership, decision-making, and supervisory skills; able to work collaboratively with staff.
- Effective communication and interpersonal skills to interact with residents, families, and external partners.
- Proficient in physical assessments, computer use, and managing the demands of the role.
