West Springfield, MA Geriatrics Physician Locum Tenens

Rhino Medical is seeking an experienced Geriatrics Physician for high-demand locum tenens coverage in West Springfield, MA .

This is meaningful, patient-centered coverage focused on complex care management, chronic disease optimization, and interdisciplinary collaboration. If you're available, scheduling is moving quickly.

• Assignment Type: Geriatric Medicine Locum Tenens
• Start: ASAP – Ongoing
• Setting: Outpatient Clinic, SNF, Assisted Living, or Hospital Consults (site-dependent)
• Patient Population: 65+ adult/geriatric patients with complex, chronic conditions
• Pay: Competitive weekly compensation
• Support: AAA Malpractice Coverage + Dedicated Credentialing + Travel & Lodging Support

• Provide comprehensive geriatric assessments and ongoing management
• Manage complex chronic diseases including dementia, heart failure, diabetes, COPD, and polypharmacy
• Conduct medication reconciliation and deprescribing when appropriate
• Coordinate care with interdisciplinary teams including nursing, social work, therapy, and family caregivers
• Address mobility, fall risk, cognitive decline, and functional status changes
• Document encounters accurately and efficiently within the EMR

• Cognitive Care: Dementia evaluation and management, delirium assessment, memory care support
• Chronic Disease Management: CHF, HTN, diabetes, COPD, CKD
• Transitions of Care: Hospital-to-SNF coordination, discharge planning, readmission reduction
• Palliative & Goals of Care: Advance care planning, end-of-life discussions (as applicable)
• Preventive Care: Fall prevention, vaccination review, osteoporosis screening

• Degree: MD/DO
• License: Current MA license & unrestricted DEA (or ability to obtain quickly)
• Board Certification: Board Certified/Board Eligible in Geriatric Medicine, Internal Medicine, or Family Medicine (ABIM/ABFM)
• Experience: Experience managing medically complex geriatric populations preferred
