, you will be responsible for providing one-time initial patient visits to establish medical oversight and support continuity of care. You will work closely with treating providers to ensure ongoing management and remain available for provider consultation and clinical guidance as needed. • Provide one-time initial patient visits across skilled nursing facilities throughout the state
