The Discharge Planning Assistant supports the Case Management Department, working collaboratively with Inpatient RN Case Managers. Responsibilities include administrative and operational functions to organize the Care Transitions Department, supporting clinical units, interacting with patients, families, staff, and external partners.

Responsibilities
  • Support discharge functions by entering referrals, gathering clinical info for payor authorizations, and supporting post-acute transition services.
  • Assist RN Case Managers on clinical units.
  • Verify patient demographics, health care proxy, and distribute regulatory notices.
  • Assist in securing patient resources and monitor post-acute referral processes.
  • Identify in-network providers and facilitate timely discharges.
  • Utilize systems like Careport, EMR, Outlook for duties.
  • Document data accurately and communicate effectively with patients and staff.
  • Perform clerical duties such as mail handling and caller support.
  • Qualifications
    • Education: High School Diploma or GED required;
      Associates or Bachelor’s preferred.
    • Experience: Proficiency with computer systems (Meditech, Epic, Cerner, Outlook), excellent communication and problem-solving skills, healthcare experience (minimum 2 years preferred), knowledge of medical terminology and community resources.
    • Certifications: BLS required.
