Job Description :

Job Summary: The Med to Bed program currently services our patients being discharged from the medical center. It helps to identify and address any medication access barriers to ensure that patients have their medications and are counseled on appropriate use prior to their discharge. The primary goals of this service are to help facilitate a smooth transition out of the hospital, promote discharge therapy adherence and help prevent non-adherence related readmissions.

A Transitions of Care Pharmacist will be responsible for the quality assurance of the pharmacy services to patients discharging from the Medical Center. This pharmacist will ensure the correct medications are dispensed to the correct patients in an efficient and safe manner as promised. This position is a part of the interdisciplinary team that includes clinical pharmacists, case management, prescription assistance supervisors, and medical providers.

This role will be based out of BILH Pharmacy and may fulfill all of the job responsibilities of a retail staff pharmacist.

Essential Responsibilities:

  • Assists with the medication reconciliation process at discharge in coordination with the interdisciplinary care team to prevent any medication omissions, dosing errors, continuation of incorrect medications, therapeutic duplications or clinically significant drug interactions.
  • Collaborates with the inpatient care team and interdisciplinary team to help identify and address any medication access barriers to optimize medication regimen prior to discharge.
  • Provides medication education and counseling to patients as part of Med to Bed program operations, in order to optimize patient's transition of care.
  • Actively coordinate post-discharge follow up related to medications for high-risk patients to aid in reduction of readmission.
  • Performs quality assurance of prescriptions and medications to dispense.
  • Performs clinical assessments and reassessments and performs and documents clinical interventions per protocol. Reviews pertinent laboratory values and assessments to determine appropriateness of medication.
  • Dispenses prescribed medications, drugs, and other pharmaceuticals for patient care: reviews prescriptions issued by physician, or other authorized prescriber to assure accuracy; directs pharmacy staff engaged in packaging, and labeling pharmaceuticals; answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage and storage of pharmaceuticals.
  • Identifies and addresses medication related barriers at the point of transition from hospital to community.
  • Participates as a member of various Quality Management Committees to provide quality management oversight to the pharmacy operations and client satisfaction.
  • Oversight of pharmacy technicians/interns and workflow; intervenes and follows protocol when necessary to correct information. Instructs interns and other medical personnel on matters pertaining to pharmacy.


Required Qualifications :

  • Bachelor's degree required. Doctoral degree in Pharmacy (Pharm

    D) preferred.
  • License Massachusetts Pharmacist required.
  • 1-3 years related work experience required.
  • Possesses knowledge in drug information, disease state management, and legal requirements of pharmacy practices.
  • Understanding of insurance and adjudication processes.
  • Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases.


Preferred Qualifications :

  • 1-2 years of experience in transitions of care.
  • ASHP Accredited Residency in Pharmacy Practice (PGY1).
