This position is eligible for a $3000 INCENTIVE for joining GHC! (Amount prorated based on FTE)
ResponsibilitiesThe Care Team Specialist (CTS) participates as a member of a care team and supports high quality patient care including engaging patients in maintaining their physical and mental well-being. The CTS performs technical, clinical, and administrative tasks which contribute to the smooth workflow and quality patient care experience in the clinic. Responsibilities include, but are not limited to:
- Pre-visit preparation
- Monitoring patient flow
- Rooming patients
- Obtaining patient histories and vital signs
- Managing certain test results following protocols
- Supporting Epic in-basket workflow
- Returning phone calls
- Assisting with medical examinations and procedures
- Assisting and collaborating with Registered Nurses and Medical Providers in patient care management and coordination
- Participating in the review of team/provider in-baskets and MyChart messages for the assigned care team
- CTS staff may receive delegated assignments from Practitioner and Registered Nurse staff
- CTS staff at GHC-SCW will have completed medical assistant training, licensed practical nurse training, Paramedic training, or EMT training from an accredited program
- Must meet the requirements for a Medical Assistant (MA), a Licensed Practical Nurse (LPN), or an Emergency Medical Technician (EMT)
- Medical Assistants must have successfully completed an MA training program from an accredited school or program and must be eligible for and receive national certification within six (6) months of employment, or have three years of MA experience with current national certification. Certification must be maintained through CME requirements, from one of the following organizations: CCMA, AAMA, ARMA, NCCT, or an approved accredited organization with CME requirements
- Licensed Practical Nurses must have completed an LPN training program and hold current licensure as a practical nurse in Wisconsin or obtain licensure within six (6) months of employment
- Emergency Medical Technicians must have completed an approved training program and hold a valid State of Wisconsin license as EMT, AEMT, or Paramedic (EMT-P)
- Possession of or eligibility for a valid Healthcare Provider CPR/AED certificate is required. Certification must be obtained within four (4) months of employment
- Ability to travel to various work sites and provide personal transportation. Possession of a valid Wisconsin driver license if operating a motor vehicle
- One year of relevant experience working in a clinic or outpatient setting is desirable
- Knowledge of and ability to perform or learn standard clinical procedures according to established protocols and policies
- Knowledge of and/or ability to learn how to properly use and maintain equipment, materials and supplies used during treatment
- Knowledge of and/or ability to learn and use computer-based applications such as MS Office, electronic medical record systems and scheduling applications. EPIC experience is desirable. Typing speed of at least 30 WPM with minimal errors
- Knowledge of outpatient clinical administrative procedures
- Knowledge of medical terminology and basic pharmacology
- Ability to maintain quality, safety and infection control standards
- Ability to recognize, identify and report problems to appropriate personnel
- Basic understanding or ability to learn reimbursement/insurance practices for medical care
- Knowledge of inventory management practices
- Ability to instruct patients and/or families on the basics of disease and proper care, treatments and follow-up
- Knowledge of medical records and appropriate documentation
- Knowledge of interviewing practices and techniques to gather information
- Excellent written and verbal communication skills; ability to communicate respectfully with patients, family members, visitors, and the healthcare team
