About the role

The purpose of Casa’s Reentry Program is to reduce relapse and recidivism among Latino reentering citizens living with co-occurring substance abuse and mental health disorders (CODs) by expanding access to substance use disorder treatment and related recovery and reentry services. Under the direction of the Project Director, they will be responsible for coordinating with correctional facilities to transition participants from corrections to a recovery-oriented continuum of care and developing and strengthening system linkages to expand access to the community resources necessary to lay the groundwork for change, support sustained recovery, and facilitate meaningful participation in the community.

The Reentry Coordinator will provide supervision and oversight to Reentry Case Managers and work in collaboration with other Casa programs to develop and strengthen system linkages to enhance and expand access to a recovery-oriented continuum of care, as these are critical to their stability and behavioral health engagement. The Reentry Coordinator will ensure that benchmarks, timelines, goals, and objectives are being met and reported as required.

As part of the Casa Esperanza, Inc. team, they will play a lead role in providing services in a fully integrated, holistic, and collaborative manner using a team-treatment approach to ensure the coordination of all client services, both across Casa programs as well as with external providers.

What you'll do Program Planning and Service Delivery
  • Coordinate and conduct regular outreach to spaces frequented by active users in the community and service providers. treatment providers, correctional facilities, parole/probation boards, and other community-based organizations
  • Represent the agency by providing education and participating in community events, including monthly Reentry Panels in correctional facilities
  • Coordinate psycho-education groups and re-entry services inside correctional facilities and/or through telehealth for current and potential patients
  • Build collaborative relationships within the community that support a strong recovery system of care, including linkages to bilingual self-help supports and services
  • Work with residential staff to coordinate internal and external services, i.e. transportation, legal, medical, and housing, employment, and training, etc.
  • Recruit community stakeholders to participate in the Latino Recovery Network to develop a comprehensive, multi-agency approach to improving reentry services and co-facilitate quarterly meetings.
  • Continue to develop a sustainable system of care focused on Reentry services and supports
  • Review pre-release assessments, including screenings for co-occurring disorders, overdose risk, and recidivism risk to determine eligibility
  • Manage referral process and monitor case assignments to ensure appropriate caseloads
  • Ensure Reentry Specialists collaborate with Leadership and Staff in correctional facilities to develop transition plans for patients leaving correctional facilities and entering Casa’s system of integrated care
  • Consult with parole officers, providing all necessary information for the parole supervision plan
  • Advocate for Reentry services, wrap‑around supports and community care to specialty courts, parole, probation, DCF, DMH and other state‑wide agencies in accordance with Casa policies
  • Collaborate with Leadership to address any barriers to implementation or programs & services
Administrative
  • Ensure program is meeting state and federal standards of care and reporting requirements
  • Assist in data collection and outcomes reporting, per grant guidelines, parameters and regulations
