Qualifications and Desired Experience:

• Minimum of 1 year certified home care nursing experience
• At least 1 year of case management experience
• Ability to travel to patient cases as assigned throughout the service area
• Proficient in electronic documentation and interdisciplinary communication
• Strong skills in patient assessment, care plan implementation, and patient/family education

Responsibilities:

• Conduct comprehensive patient assessments in home settings
• Develop and implement individualized care plans based on patient needs
• Provide education and support to patients and their caregivers
• Perform admission procedures and manage cases per organizational policies
• Maintain accurate and timely electronic medical records
• Participate in interdisciplinary team communications to optimize patient care
• Rotate weekend shifts approximately once every six weeks
