An Adult Day Health Center is seeking a Health & Social Services Coordinator to join their staff in Boston, MA. In this role, you will support participants by linking them to community services, completing assessments, and helping manage their care plans. The position offers an estimated pay range of $19-$21 per hour, with a Monday-Friday schedule and one Saturday per month.

- Monday-Friday, 8:00 AM-4:00 PM
- One Saturday per month (9:00 AM-3:00 PM)
- Support a new organization dedicated to senior care and connect participants with vital community resources
- Build meaningful relationships with seniors and their families
- Work in a supportive, collaborative environment with opportunities for professional growth
- Contribute to improving the quality of life for participants and their families
- Inform and refer participants to appropriate community services, assisting with applications and transportation as needed
- Complete comprehensive assessments and develop care plans
- Document service coordination activities accurately in health records
- Coordinate physical, mental, and behavioral services during enrollment, treatment, and discharge
- Assist with personal, social, and family issues affecting participants
- Collaborate with family members and community agencies to ensure comprehensive care
- Assess, plan, and evaluate services to meet participants' needs
- Minimum one year of recent experience working professionally with adults
- Ability to read, write, and communicate fluently in English and Spanish
- Pre-employment health screening certifying suitability for the role
- Minimum one year of recent experience working professionally with adults
- Bachelor's degree in human services or a related field from an accredited college or university
- Ability to read, write, and communicate fluently in English and Spanish
- Experience working with seniors, especially those with memory loss, and demonstrating compassion for this population
- Pre-employment health screening certifying suitability for the role
