- Provide comprehensive physical therapy evaluations, treatment planning, therapeutic interventions, and home safety assessments for older adults enrolled in the PACE program
- Deliver care in both the Adult Day Health Center and participants' homes throughout the service area
- Develop individualized plans of care focused on maintaining independence and preventing functional decline
- Supervise Physical Therapist Assistants (PTAs) and rehabilitation aides as needed
- Collaborate with an interdisciplinary PACE team while maintaining autonomy in clinical decision-making
- $84,000-$113,000 Annual Salary
- Mileage Reimbursement
- Monday-Friday
- 8:00 AM-4:00 PM
- Approximately 6 Patients Per Day
- No Evenings
- No Weekends
- No On-Call Responsibilities
