North Suffolk is looking for a Housing Specialist to join our team! The Housing Specialist provides direct care services at the site-based and mobile Emergency Respite Program.
This paraprofessional position also provides direct services to people in respite setting in the areas of daily living skills, transportation, skill-building, money management, medication administration, risk mitigation and other areas based on the person-centered treatment plan. Services are provided based on recovery tenets and are aimed to promote overall health and well-being of persons served
Schedule:
Tuesday - Friday 2pm-10pm. Saturday 8am-4pm.
- Develop and maintain expertise in assisting persons served with all aspects of housing search: identification of barriers to housing and overall assessment of client-specific housing options.
- Acts as liaison with, staff, family members, advocates, and local, state, and federal housing staff and programs as needed related to matters of housing.
- Document each person’s progress in the confidential record according to established methods and procedures.
- Organize, update, and oversee the unit’s housing resource board to ensure residents have access to current information on community housing options and housing eligibility requirements.
- Meet with all mobile and site-based Emergency respite clients upon admission and complete overall assessment of their housing needs and assist client in establishing immediate and longer-term goals for client. Communicate to other team members and assign/delegate tasks as needed to other team members including staff and trainees.
- Education:
- High School Diploma/GED equivalent required.
- Bachelors Degree highly preferred.
- Experience:
- At least one year working in case management and/or housing community/coordination
- Highly preferred, not required:
At least one year experience with population that has severe and persistent mental illness and/or substance use disorder;
Experience working with local, state, and federal housing programs;
Individuals with lived-experience
- Skills:
- Must have Driver’s license.
- Must be MAP certified and CPR/1st aide certified within the first five months of hire (training provided).
