Boston Children's Hospital is seeking a Clinical Social Worker to join our Emergency Department overnight team. In this role, you will provide crisis-oriented psychosocial assessment, counseling, and discharge planning for pediatric patients and their families in a fast-paced, Level I trauma environment. You will conduct risk and safety assessments, coordinate with medical and psychiatric providers, and connect families with critical community resources.
Working on-site in Boston, MA, you'll be part of a collaborative, mission-driven team dedicated to trauma-informed, family-centered pediatric care and continuous learning.
Responsibilities
- Provide crisis-oriented psychosocial assessment and counseling to pediatric patients and families in the Emergency Department during overnight shifts
- Conduct suicide risk, safety, and abuse/neglect assessments and coordinate mandatory reporting as needed
- Collaborate with physicians, nurses, psychiatry, and care management to develop and implement safe discharge and follow up plans
- Facilitate family meetings, care conferences, and resource navigation for high risk and vulnerable families
- Coordinate referrals to community mental health, social service, shelter, and protective agencies
- Document assessments, interventions, and care plans in the electronic medical record in a timely, accurate manner
- Participate in interdisciplinary rounds and contribute to care planning for behavioral health and medically complex patients
- Support trauma informed care, de escalation, and grief and bereavement support in acute situations
- Provide crisis intervention and support for staff following critical incidents
- Engage in ongoing professional development, supervision, and quality improvement initiatives
