Social Worker, Crisis Counselor

RCM Healthcare Services seeks a licensed clinical Social Worker to provide high-quality psychosocial care across partner hospitals, clinics, and community settings. In this mission-driven role, you will complete assessments, deliver therapeutic interventions, coordinate discharge planning, and connect patients with vital community resources. Responsibilities

• Provide psychosocial assessments and evidence-based counseling to patients and families
• Develop, implement, and monitor individualized care and discharge plans
• Coordinate community resources, referrals, and support services
• Collaborate with interdisciplinary healthcare teams across client facilities
• Maintain accurate, timely clinical documentation and case notes
• Support crisis intervention and risk assessments as needed
• Advocate for patient rights, access to care, and continuity of services
• Participate in quality improvement and compliance with regulatory standards
