The Social Worker serves as an advocate and resource for residents and families by addressing psychosocial needs, supporting transitions of care, and collaborating with clinical teams to promote resident quality of life.
This position is essential in helping residents maintain independence, dignity, and emotional well-being while ensuring their needs and preferences remain at the center of care.

Responsibilities Responsibilities Resident Support & Advocacy- Complete psychosocial assessments and identify residents’ emotional, social, and psychological needs
- Develop and implement individualized care interventions that support resident goals and quality of life
- Provide support and guidance to residents as they adjust to the nursing home environment
- Advocate for resident rights, dignity, independence, and personal choices
- Provide emotional support during challenging transitions and life events
- Assist residents and families with accessing appropriate community resources and services
