Physical Therapist

We are seeking licensed Physical Therapists to provide personalized, in-home and community-based care to older adults in various locations across the Greater Boston area.

Key Responsibilities Include

• Performing evaluations, creating individualized treatment plans, and delivering evidence-based therapy to support mobility, independence, and overall well-being.
• Providing one-on-one care in private residences and senior living communities, and utilizing telehealth when appropriate.
• Documenting patient progress accurately within electronic medical records and collaborating with interdisciplinary teams.
• Educating patients, caregivers, and staff to promote safety and aging in place.

• Valid Physical Therapy license or eligibility in the state.
• Degree from an accredited program and proficiency with EMR systems.
• Current CPR certification.
• Ability to meet physical demands, including lifting, repositioning, and assisting patients, along with driving to patient locations.
