The Care Coordinator is responsible for assisting clients to obtain needed services by providing information, referral coordination and follow-up. The Care Coordinator reports to and is supervised by the Clinical Director or his/her designee for weekly supervision and programmatic assignments.

Location

Women's Hope in Jamaica Plain, provides residential treatment programs for 16 women with co-occurring substance use disorder and mental health. The program is up to a year long and provides enhanced services for the residents with complex mental health concerns, as well as chronic health conditions.

Essential Functions
  • Assist clients in obtaining needed services by providing information, referral coordination, and follow-up.
  • Develop and connect with resources that address both substance use and mental health disorders.
  • Coordinate medical and behavioral health services offered through health insurance
  • Coordinate MAT (Medication Assisted Treatment) and psychiatric services while clients are in the program as well as assisting in the aftercare.
  • Provide milieu coverage, including: monitoring and assessing the integrity of the program’s therapeutic milieu; empowering clients to act in accordance with the guidelines of the program; assisting clients in self-administration of medications; conducting controlled substance counts; taking clients on fresh air breaks; answering phones and the front door.
  • Make emergency referrals (e.g. detox programs, DTA, hospital and after care placement).
  • Work collaboratively with all staff to provide individualized services to all clients.
  • Attend weekly supervision with the Clinical Director or his/her designee.
  • Maintain caseload if appropriate and/or necessary.
  • Act in accordance with Victory Programs, Inc.’s Standards of Excellence & Philosophy Care at all times, including maintaining professional ethics, boundaries and client confidentiality and working from a strength based perspective using evidence based practices (MI,CBT, Harm Reduction etc.)
  • Ensure that all services are delivered in compliance with agency standards and contractual requirements.
  • Ensure all policies and procedures of Victory Programs, Inc. are being followed.
  • Perform other duties or tasks that may be requested by a Supervisor or other designated personnel.
Qualifications
  • Bachelor’s degree required. May substitute for 3 years’ experience working in a residential setting, preferably with a co-occurring population. Master’s preferred.
  • Strong organizational and record keeping skills.
  • Ability to work independently and cooperatively as a member of a professional team.
  • Ability to relate to clients in an instructive, directive, firm yet compassionate manner.
  • Knowledge of the addiction recovery process and/or mental health conditions, or a willingness to acquire it within the first six months of employment.
  • Experience working with those experiencing homelessness.
  • Demonstrated ability to work successfully in multicultural settings with diverse client groups.
  • Proficient computer skills, including the use of Microsoft Word and Excel.
  • Strong organizational, writing and communication skills.
  • Must be CPR certified (may be obtained upon hire)
  • Completed TB screening (may be obtained upon hire)
  • Three work related professional references; at least 2 from manager or supervisor.
Position Type

Full-time (40 hours) Monday-Friday 9am-5pm. On-site

Rate

$26.00/hr
