About the Role:As a Hospital Liaison, you will serve as the primary liaison between our healthcare facility and local hospitals, ensuring seamless communication and coordination of patient care. You will play a vital role in facilitating the transition of patients between our facility and the hospital, while maintaining open lines of communication with medical staff, care teams, and family members.
Responsibilities:- Establish and maintain relationships with local hospitals, physicians, and other healthcare providers to ensure effective communication and coordination of patient care
- Verify and coordinate patient admissions, discharges, and transfers between our facility and local hospitals
- Facilitate the exchange of medical information, test results, and treatment plans between healthcare providers
- Serve as a liaison for patient families and care teams, addressing concerns and providing education on patient care and treatment plans
- Monitor patient flow and communicate with hospital staff to ensure timely and efficient care transitions
Participate in quality improvement initiatives to optimize patient outcomes and facility operations
- 2 years of experience in a hospital or healthcare setting
- Excellent communication, interpersonal, and problem-solving skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Strong understanding of healthcare operations and patient care coordination
- Compassionate and empathetic approach to patient care and family support
