You will be responsible for fostering strong relationships with our partners, coordinating patient transfers, and providing high-quality patient care. Your expertise will enable us to deliver exceptional patient outcomes while promoting efficient and cost-effective care delivery.

• Develop and maintain strong relationships with referring hospitals, healthcare providers, and other stakeholders to ensure effective communication and coordination of patient care
• Conduct patient assessments and develop personalized care plans to ensure optimal outcomes
• Coordinate patient transfers, including arranging transportation and communicating with patients, families, and healthcare providers
• Collaborate with interdisciplinary teams to ensure comprehensive care and address any issues or concerns
• Participate in quality improvement initiatives to enhance patient care and satisfaction
• Maintain accurate and timely records of patient interactions and communications
