Signify Health (a CVS Health affiliate) is hiring part‑time board‑certified nurse practitioners to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and some commercial plan members. You will travel locally, primarily within a metro area, and perform 45‑minute visits including history review, physical evaluation, and optional DPS tests.
Expect 12 hours of weekly availability with 50+ units per month and 35+ visits monthly.
