You will coordinate outreach to shelters and congregate facilities, supervise staff, and support data collection for epidemiologic activities.
Responsibilities include care navigation, directly observed therapy, education, and collaboration with partners to advance racial equity and health outcomes. Must hold MA RN licensure and BSN.
