Position: Assistant Unit Coordinator for Subacute Unit LPN or RN

Overview

Subacute Assistant Unit Coordinator (LPN or RN) – St. Joseph Rehab and Nursing Center is looking for a Full-time Subacute Unit Charge Nurse, Monday - Friday, to join our team. This is a nursing administrative support role requiring attention to detail, strong communication skills, and the ability to navigate the operations of a busy, subacute environment. The Subacute Unit Charge Nurse will be interacting and communicating with physicians, families, fellow staff members, vendors, and residents.

This is a great opportunity for someone who is looking to continue to keep the interaction with residents and nursing skills sharp yet work a Monday - Friday schedule in an administrative capacity.

Job Responsibilities of the Subacute Unit Charge Nurse (LPN or RN)
  • The primary role of this function is to manage both communication and workflow so the resident, family and the physician obtain accurate and updated information as soon as possible. Interacts with residents, families, the interdisciplinary team, and medical professionals in a positive and friendly manner.
  • Manages the Subacute Unit Nursing Station and its functional desk duties, including providing informational needs to all parties and is the liaison between patients, families, and their PCPs.
  • Assists in managing appointments, transportation, and communication flow of the short-term rehabilitation unit. Provides direct activities that involve management of the electronic health record (EHR) consisting of entering, transcribing, and processing physician’s orders, ensuring that the electronic medication administration and treatment records are up to date and accurate. Ensures that Subacute Unit programs and systems are in compliance such as:
    • C.N.A. documentation is completed each date and assesses its accuracy. Reports to the Unit Manager any inaccuracies for correction.
    • Clinical dashboard in the EHR daily to ensure scheduled assessments are completed timely as delegated by the Unit Manager.
    • Reviews Hospital Transfer Log to ensure completeness and accuracy of transfer information.
  • Maintains resident appointment scheduling and collaborates with the Unit Manager to ensure that only necessary appointments during the resident’s stay are provided by the facility.
  • Maintains center’s Ambulance Log and provides to the Administrator monthly. Communicates to the physician and resident’s responsible person when there is a change in condition that affects resident’s plan of care.
  • Collaborates with Rehab and Social Services identified post-discharge appointment needs to ensure scheduling as required.
  • Ensures that necessary nursing information related to change of condition and treatment plans are communicated to the team during Nursing Huddles.
Knowledge, Skills, Qualifications, Experience, Education
  • Graduate from an accredited school of Nursing.
  • Licensed RN or LPN in Massachusetts.
  • At least one (1) year working as a Licensed Practical Nurse or Registered Nurse.
  • Demonstrates the ability to read, write, and speak English, as well as add, subtract, multiply and divide.
  • Proficiency with computer skills including Microsoft Office and an EMR (PCC) preferably.
