The Registered Nurse (RN) at Elmwood Healthcare - Home Health will be responsible for delivering comprehensive nursing care to patients in their homes. You will perform a range of clinical tasks, coordinate care with other healthcare professionals, and provide education and support to patients and their families.

Field-Based

Key Responsibilities:

• Patient Care: Provide direct nursing care, including administering medications, wound care, IV therapy, and other treatments as prescribed by the physician.
• Assessment and Evaluation: Conduct comprehensive assessments of patients' health status, including vital signs, medical history, and physical examinations.
• Care Coordination: Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure a cohesive approach to patient management.

  Educate patients and their families on disease management, medication administration, self-care techniques, and lifestyle modifications.
• Documentation: Maintain accurate, detailed, and timely documentation of patient care, including progress notes, treatment plans, and any changes in patients' conditions.
• Health and Safety Compliance: Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment.
• Emotional Support: Provide compassionate support to patients and their families, addressing concerns and offering comfort and reassurance as needed.

• Infusion Therapies (Excluding IV Push)
• Drain Care and Management
• Catheter Care and Management
• Ostomy Care and Management
• VAD Management and VAD Site Care (Excluding Discontinuation)
• Venipuncture and Blood Sample Collection via VAD

Education: Associate's, Bachelor's Master’s or Doctoral degree in Nursing from an accredited program.

Licensure: Current and valid license to practice Registered Nursing (RN) in the Commonwealth of Massachusetts.

Experience: Minimum of one (1) year of experience as a Registered Nurse (RN), preferably in a home healthcare setting.

Skills: Strong clinical assessment and therapeutic skills, excellent communication and interpersonal skills, ability to work independently and manage time effectively.

Certifications: CPR Certification with Basic Life Support (BLS) or Advanced Life Support (ALS) is required.

Other Requirements: Valid driver’s license, reliable transportation, and willingness to travel to patients' homes within the designated service area.
