We are seeking a compassionate, detail-oriented Registered Nurse (RN) – Acute Rehab to join our team. This role is responsible for delivering high-quality nursing care to patients recovering from serious illnesses, injuries, or surgeries. The RN works collaboratively with an interdisciplinary team to develop and implement individualized care plans aimed at maximizing patient recovery, independence, and overall well-being.
Key Responsibilities
- Provide direct patient care in accordance with the nursing process and established rehabilitation protocols
- Collaborate with physicians, therapists, and other healthcare professionals to create and implement personalized care plans
- Monitor and assess patients' conditions, document changes, and communicate updates to the care team
- Administer medications, treatments, and therapies as prescribed
- Educate patients and families on rehabilitation goals, care plans, and discharge instructions
- Promote patient safety, mobility, and participation in physical and occupational therapy
- Maintain accurate and timely documentation in electronic medical records (EMR)
- Respond to medical emergencies and perform life-saving measures as necessary
- Support a safe, clean, and therapeutic environment for patients
- Participate in quality improvement and patient safety initiatives
- Active and unrestricted Registered Nurse (RN) license in the state of practice
- Associate Degree in Nursing (ADN) or Diploma in Nursing from an accredited nursing program
- Basic Life Support (BLS) certification — Required
- Minimum 1 year of acute care or rehabilitation nursing experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Ability to work effectively in a multidisciplinary team environment
- Familiarity with EMR systems and standard nursing documentation practices
- Bachelor of Science in Nursing (BSN)
- Advanced Cardiac Life Support (ACLS) certification
- Prior experience in acute inpatient rehabilitation or a similar setting
- Certification in Rehabilitation Nursing (CRRN)
- Registered Nurse (RN) — Required
- Basic Life Support (BLS) — Required
- Advanced Cardiac Life Support (ACLS) — Preferred
- Certified Rehabilitation Registered Nurse (CRRN) — Preferred
