The Lung Transplant Nurse Coordinator (Post-Transplant) provides specialized nursing care, education, and coordination for patients following lung transplantation. Reporting to the Medical Director of the Lung Transplant Program, this role ensures continuity of care after discharge, serving as the central point of contact for patients, families, and healthcare providers. The coordinator supports patient rehabilitation, compliance monitoring, and multidisciplinary collaboration while contributing to quality assurance, research, and administrative activities.

Key Responsibilities
  • Provide outpatient care within RN scope of practice to support physical, social, and emotional rehabilitation.
  • Identify and respond to recipient/family/legal guardian’s educational, psychosocial, and economic needs.
  • Reinforce education and follow-up requirements for successful transplant outcomes.
  • Monitor and assess patient compliance with medications and medical regimens; collaborate with the transplant team to ensure adherence.
  • Collect, review, and document health and organ function data in patient charts.
  • Implement changes in drug/treatment therapy as ordered by physicians or credentialed providers. Serve as a resource for internal and external providers regarding post‑transplant protocols (e.g., high‑risk labs, NAT testing, rejection treatment).
  • Arrange consultations, diagnostic procedures, infusions, and hospital admissions/transfers as needed.
  • Maintain communication with referring physicians, PCPs, and other healthcare providers.
Program Support
  • Collaborate with medical and surgical staff to coordinate transplant care.
  • Support quality assurance and research initiatives within the transplant program.
  • Participate in administrative and financial activities related to patient care coordination.
Required Qualifications
  • Current RN license.
  • Minimum of 3 years of ICU RN experience.
  • Strong clinical assessment and patient care skills.
  • Excellent communication and interpersonal skills for patient and family education.
  • Ability to collaborate effectively with multidisciplinary teams.
  • Strong organizational skills with attention to detail in documentation and follow‑up.
Preferred Qualifications (if any)
  • Prior experience in transplant nursing or coordination.
  • Familiarity with outpatient transplant protocols and compliance monitoring.
