PD Community Care Registered Nurse

What You’ll Do

As a Per Diem Community Care Registered Nurse with Mainstay, you will play a key role in delivering high‑quality, person‑centered non‑clinical nursing support to formerly homeless individuals who are transitioning into supportive housing. Your work will directly contribute to housing stability and improved health outcomes for individuals who have been chronically unhoused. This position is part of our growing BRIDGE program.

Clients served through this program have varying levels of independence and diverse support needs, including substance use disorders and physical and/or behavioral health conditions. The role offers compassionate, respectful care while promoting autonomy and dignity. As a per diem role, hours vary week to week and typically average 4–10 hours.

• Collaborating with the program supervisor/director and support staff to provide high‑quality care.
• Conducting in‑depth assessments of potential participants and monitoring their health status.
• Building meaningful relationships with clients, their families, and caregivers.
• Ensuring compliance with Mainstay’s policies, procedures, and industry regulations.
• Evaluating and refining personal care plans to enhance client well‑being.
• Engaging in regular in‑home visits and documenting interactions and observations.

• Flexibility:
  Create your own client visit calendar and maintain a manageable caseload of approximately 4–10 hours per week.

• A Bachelor's Degree in Nursing (strongly preferred).
• 1–2 years of experience in home care or home‑based services (preferred).
• A valid RN license in MA and a valid driver’s license. Ability to travel in and around Boston, South End.
• Strong skills in electronic‑based documentation.
• Empathy, patience, and exceptional communication skills.
