Staff Nurse
Salary Range: $38.98 to $58.49 hourly. Minimum starting pay: $40.00 hourly.
Location:
Dorchester – Geiger Gibson Community Health Center. Ability to float to other centers including Daniel Driscoll Neponset Health Center.
The Staff Nurse provides nursing services at the top of the incumbent’s license to patients in accordance with all applicable Massachusetts State Laws and regulations promulgated by the Department of Public Health and Harbor Health Services, Inc. Duties include triage, access, care coordination, and nursing support of clinical sessions.
Responsibilities- Perform all nursing duties in accordance with Massachusetts Nursing Practice Act and Harbor Health policies and procedures.
- Provide routine nursing care to patients using age‑specific guidelines and standard precaution practices. Perform comprehensive assessment of patients’ health status and triage based on symptom urgency, in person and via telephone.
- Deliver patient and family teaching for all patients.
- Demonstrate a working knowledge of nursing theory, techniques, principles, and practice for holistic patient care.
- Advise patients on disease prevention, medication, treatment plans, and self‑care techniques.
- Act as liaison among the patient, care team, and/or provider.
- Provide health coaching and education for common conditions (e.g., asthma, hypertension, diabetes) through individual nurse visits, group visits, and co‑visits with other disciplines as appropriate.
- Help patients stay up‑to‑date with clinical care guidelines, immunizations, and health maintenance screenings.
- Administer medications and vaccinations as ordered by provider or per standing orders policy.
- Assess the patient’s physiological, psychological, sociological, and spiritual status.
- Promote health and wellness through education and preventative care; counsel patients on lifestyle modifications as needed.
- Document patient care activities, assessments, treatment plans, and interventions in the EHR, ensuring accurate and timely records.
- Advocate for patients, ensuring their needs and concerns are addressed.
- Support patients in navigating the healthcare system and understanding their rights and options.
- Demonstrate knowledge of medical equipment and its appropriate usage.
- Comply with all facility policies and procedures while upholding Harbor Health Services, Inc. standards.
- Maintain knowledge of all medications, reactions, documentation, and dosage requirements.
- Ensure emergency equipment in the clinical area is properly maintained and functional.
- Adhere to HHSI policy when disposing of biohazardous materials, labeling, and handling all materials.
- Participate in team, department, and clinic meetings to learn, support process improvement, and contribute to care plans.
- Successful completion of a nursing program (RN); BSN preferred.
- Current Massachusetts RN license and Basic Life Support (BLS) certification.
- Experience in a community health setting preferred.
- Experience in ambulatory care or health insurance environment preferred.
- Basic computer literacy, knowledge and experience with Microsoft Office 365 (Word, Excel, Outlook, Teams), and Electronic Medical Record (EMR) systems; EPIC/OCHIN EMR preferred.
- Excellent customer service skills and verbal and written communication skills.
- Ability to work effectively both within a team setting and independently.
- Ability to work effectively with economically, racially, and linguistically diverse customers.
- Bilingual a plus:
English and Vietnamese/Spanish/Portuguese or Haitian Creole.

Our health centers are open 1 or 2 evenings a week (Tuesdays and Thursdays) and Saturdays, 8:30 AM – 12:30 PM at Neponset Health Center. Shifts are 8:00 AM – 5:30 PM or 10:45 AM – 8:30 PM Monday through Friday. Per diem (on call) as needed to cover open shifts.
