About This Role
The Nurse Navigator for Palliative Care at Brigham and Women’s Hospital serves as a central coordinator for managing referrals, patient flow, and care transitions for patients with serious or complex illnesses. This role is ideal for experienced nurses who excel in communication and collaboration within interdisciplinary teams.
Highlights- Pay: $41.71 - $105.65/Hourly
- Location: Boston, MA
- Facility: Brigham and Women's Hospital
- Schedule: Full-Time, Rotating Shift
- Manage inpatient palliative care consult referrals from various sources.
- Collaborate with referring clinicians to obtain clinical and psychosocial information.
- Prioritize referrals based on symptom burden, acuity, and goals-of-care needs.
- Facilitate communication among interdisciplinary teams and specialty services.
- Participate in interdisciplinary huddles to assess patient appropriateness for care pathways.
- Educate clinicians and staff on pathway criteria and monitor patient progression.
- Gather clinical and social information to determine appropriate service lines and care settings.
- Document patient care in compliance with nursing documentation standards.
- Develop comprehensive plans of care and discharge plans utilizing appropriate resources.
- Participate in quality management and improvement programs.
- Graduate of an approved school of nursing with current registration in Massachusetts required. BSN required for newly licensed nurses.
- Minimum of 3 years of nursing experience with complex care populations required.
- Strongly prefer experience in palliative care.
- Prefer inpatient experience in oncology, geriatrics, or emergency medicine.
- Strongly prefer prior experience in nurse navigation, care coordination, or transitions of care.
- Registered Nurse in Massachusetts required.
