Position OverviewWe are seeking a compassionate, motivated, and clinically skilled Nurse Practitioner (NP) to join our growing primary care practice on a full-time or part-time basis. The ideal candidate will provide comprehensive outpatient care to adult and/or pediatric patients while collaborating with physicians and clinical staff to deliver high-quality, patient-centered care.
This position is ideal for a provider who is comfortable working in a fast-paced environment and values preventative medicine, chronic disease management, and continuity of care.
Responsibilities Clinical Duties- Perform comprehensive patient evaluations, physical examinations, and medical histories
- Diagnose and treat acute and chronic medical conditions
- Develop and implement treatment plans
- Order, interpret, and review laboratory and diagnostic testing
- Prescribe medications and therapies in accordance with New York State regulations
- Provide preventative care, wellness visits, and patient education
- Manage chronic conditions including:
- Hypertension
- Type 2 Diabetes
- Obesity
- Hyperlipidemia
- Perform documentation accurately and efficiently within EMR/EHR systems
- Collaborate with physicians, medical assistants, and administrative staff to ensure smooth patient flow
- Maintain timely chart completion and documentation compliance
- Participate in quality improvement and patient satisfaction initiatives
- Assist with medication prior authorizations and care coordination when needed
- Master’s or Doctorate degree from an accredited Nurse Practitioner program
- Active New York State Nurse Practitioner license
- Board certification as:
- Family Nurse Practitioner (FNP), or
- Adult-Gerontology Primary Care NP (AGPCNP)
- Active DEA registration (or eligibility)
- Minimum 1 year Primary care experience preferred (new graduates may be considered depending on training and background)
- Experience with outpatient EMR/EHR systems
- Strong communication and patient education skills
- Bilingual candidates encouraged to apply (Spanish, Portuguese, etc.)
- Experience with:
- Annual wellness visits
- Weight management
- Preventative medicine
- Women’s health
- Men’s health
- Ability to work independently while collaborating with supervising physicians
- Strong organizational and multitasking abilities
- 4–5 days per week
