Nurse Practitioner About the Role

We’re seeking a Nurse Practitioner to join a collaborative primary care team in a full-time outpatient setting. This role offers the opportunity to provide comprehensive care for patients of all ages, with a focus on quality, continuity, and patient-centered treatment.

• Perform patient histories, physical exams, and preventive screenings
• Diagnose and manage acute and chronic conditions across diverse populations
• Order, interpret, and follow up on diagnostic tests
• Collaborate closely with physicians, nurses, and specialists to coordinate care
• Document all patient interactions accurately in the EMR
• Participate in quality improvement initiatives and team-based care planning
• Help connect patients to community health and wellness resources

• Active and unrestricted Nurse Practitioner license
• Current national board certification and DEA registration
• CPR certification
• Experience in outpatient or community health settings preferred
• Strong clinical judgment and the ability to work independently and as part of a team
