Position: RN Float Case Manager, 20 Hours (Days / 2x10 Hour Shifts / Every Weekend)

Position Summary

Utilizing a collaborative process, the care manager will assess, plan, implement, monitor, and evaluate the options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost‑effective outcomes. The care manager helps identify appropriate providers and facilities throughout the continuum of services while ensuring that available resources are used in a timely and cost‑effective manner to obtain optimum value for both the patient and the reimbursement source.

Responsibilities
  • Asses and plan patient care interventions.
  • Implement, monitor, and evaluate care plans.
  • Identify and coordinate with appropriate providers and facilities.
  • Ensure timely and cost‑effective use of resources.
  • Support the overall care management process and department.
Job Requirements
  • Education:

    Graduate of an accredited BS program in Nursing.
  • Licensure:
    Licensed to practice as a Registered Nurse in Massachusetts.
  • Preferred Certification:
    Care‑Management‑Certification (CCM) or equivalent within 24 months of hire.
  • Experience:

    3–5 years of diversified clinical experience, including at least 2 years in a utilization/case‑management role involving judgment and decision‑making.
Knowledge and Skills
  • Extensive background and knowledge of current medical/surgical patterns of practice.
  • Medical terminology and standard medical abbreviations.
  • Managed‑care concepts and health‑care delivery systems.
  • Assessment of physical/mental impairment, disease characteristics, and expected treatment.
  • Confidentiality issues and legal/ethical issues.
  • ICD‑9 and CPT nomenclature and interpretation.
  • Benefits, plan contracts, eligibility issues and requirements.
  • Case‑management program philosophy and responsibilities.
  • Excellent interpersonal, verbal, written, and negotiation skills.
  • Strong analytical, data‑management, and PC skills.
Compensation and Benefits

Compensation Range: $38.05 – $75.33 per hour. Total compensation includes medical, dental, vision, pharmacy benefits, contract increases, Flexible Spending Accounts, 403(b) savings matches, earned time cash out, paid time off, career advancement opportunities, and resources to support employee and family well‑being.
