Clinical Coordinator
In-Office | Braintree, MA | Full Time (No weekends or On-Call)

- Support quality care delivery by promoting Elara Caring’s mission and clinical standards
- Review and process coordination notes and clinical workflow tasks in Home Care Home Base (HCHB)
- Obtain and enter physician orders (verbal, written, and faxed) for clinical review and approval
- Maintain and update patient tracking tools, including at-risk registries
- Assist with scheduling, follow-ups, and communication between patients, caregivers, and care teams
- Help obtain prior authorizations and support administrative clinical needs
- Conduct chart audits to ensure compliance with company policy and regulatory standards
- Verify required patient notices and documentation (including Medicare-related forms)
- Maintain strict confidentiality in accordance with HIPAA guidelines
- Provide occasional support with field visits or direct patient care, as needed
- Perform additional duties to support the clinical team and patient care operations
- Graduate of an accredited Licensed Practical Nursing program
- Active, unrestricted LPN license in Massachusetts
- Minimum 1 year of nursing experience in a clinical setting
- Strong attention to detail and organizational skills
- Comfortable working in an office-based clinical coordination role
- Ability to travel within the branch coverage area if needed
- Valid driver’s license, insurance, and reliable transportation (if travel is required)
- Ability to sit, stand, bend, lift, and move intermittently; able to lift 50–100 lbs as needed
