As a Weekend LPN, you will provide direct patient care, administer medications, and assist with daily living activities in our state‑of‑the‑art facility. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring that our patients receive the highest level of care possible.

• Provide direct patient care, including vital sign assessments, wound care, and medication administration
• Assist with daily living activities such as bathing, dressing, and grooming
• Maintain accurate and detailed patient records
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Provide education and support to patients and families
• Monitor and report any changes in patient condition to the nurse manager or physician

• Current LPN license in the state of Massachusetts
• Minimum 1 year of experience in a healthcare setting
• Ability to work a variety of shifts, including weekends and holidays
