Summary
The Patient Care Assistant II functions as a member of the patient care team, under the direction of the Registered Nurse (RN), and consistently performs all aspects of the PCA role, supporting the RN with clinical tasks such as blood glucose monitoring, completing EKGs, applying simple dressings, using aseptic technique, and responding to medical emergency situations utilizing Heartsaver CPR and AED skills.
The PCA II also responds to the needs of patients and family members and takes an active role in the efficient operation of the unit, ensuring the practice of the Patient and Family Centered Care Model.
- Anticipates and identifies patient safety risks in cooperation with the licensed nurse and takes appropriate action to reduce or eliminate risks.
- Makes routine patient room rounds to address patient needs.
- Follows restraint policies under the supervision of a licensed nurse and checks with the nurse before utilizing restraints.
- Provides 1:1 monitoring as a patient observer for patients at risk for injury.
- Follows hospital infection control and safety procedures.
- Utilizes proper body mechanics when moving and assisting patients to prevent personal injury.
- Maintains and promotes an atmosphere that reflects the mission and values of the organization.
- Respects and supports a culturally diverse staff and patient population.
- Delivers care in a non‑judgmental, non‑discriminatory manner that is sensitive to patient diversity.
- Maintains confidentiality and privacy of patient, employee, and hospital information.
- Works cooperatively with all staff members and seeks assistance from RN, Nursing Director or Nursing Supervisor when needed.
- Dresses appropriately and follows dress code requirements, including wearing a visible hospital I.D. badge.
- Organizes time efficiently and prioritizes patient care needs, working with other PCAs to facilitate care.
- Provides assistance to co‑workers when requested.
- Adapts to changing patient and unit needs, including offering assistance, floating, and adjusting assignments.
- Communicates with Charge Nurse/Nursing Supervisor to coordinate break periods and maintains visibility on the unit without leaving the work area without notification.
- Reviews and is knowledgeable of appropriate policies, procedures, and work rules.
- Understands hospital emergency codes and how to call a code.
- Uses hospital e‑mail to access staff meeting minutes, notices, and memos.
- Participates in staff meetings and appropriate in‑services.
- Maintains timely verbal communication with RN during the shift, notifying them immediately of significant changes in a patient’s condition or monitor alarms.
- Accurately collects and records patient data (vital signs, intake and output, blood glucose, weights, etc.) and complies with documentation standards for a complete record.
- Maintains and fosters a teamwork approach to patient care.
- Provides instructive, non‑clinical information to patients and families, orienting them to hospital and unit services.
- Distributes patient mail, flowers, and essential personal care items and ensures accessibility of call lights.
- Runs patient errands and other duties as assigned by the RN.
- Reinforces patient education from the RN and gives basic instructions on using the call light, positioning, transferring, walking, toileting, and feeding.
- Complies with established practice standards, policies, procedures, protocols, guidelines, and regulations such as DPH, JCAHO, Boston fire codes, and OSHA.
- Maintains competence in technical skills including application of external monitors, use of a CPM machine, performing EKGs, blood glucose monitoring, simple dressings, and Heartsaver CPR and AED skills.
- Assists in the orientation of new PCAs and serves as a floating staff resource.
- Participates in hospital‑wide and unit‑based SEAQ programs and activities.
