RN Pharmacy & Medication Coordinator | Skilled Nursing

A skilled nursing facility in eastern Massachusetts is seeking an experienced RN Pharmacy & Medication Coordinator for a full-time, non-bedside role offering $105,000-$110,000 annually with no clinical on-call responsibilities.

This position focuses on medication coordination, therapeutic interchange processes, and collaboration with pharmacy providers and physicians to support safe, efficient, and compliant medication management for residents.

• Ensure medications align with facility formulary and medication profile
• Coordinate therapeutic interchange processes with pharmacy and physicians
• Facilitate prior authorizations and medication adjustments
• Communicate medication updates with the interdisciplinary care team
• Support compliance with medication-related regulations
• Maintain accurate and timely documentation

• Active RN license required
• Experience in skilled nursing, post-acute care, pharmacy coordination, or medication management preferred
• Strong organizational and communication skills
• Detail-oriented with the ability to manage multiple workflows
