Brockton Home Health Care AFC is looking for a Registered Nurse (RN)/Case Manager to manage and support our Adult Family/Foster to make sure the our programs runs smoothly.

About the Position:
We are seeking a caring and detail-oriented Registered Nurse to join our Adult Foster Care (AFC) program as an Initial Assessment / Admission Nurse. In this role, you will complete in-home evaluations for individuals applying to the AFC program, reviewing their needs and confirming eligibility. This opportunity is ideal for nurses with experience in geriatrics, home health, or community-based care who are committed to supporting individuals and enhancing quality of life.

Key Duties:

  • Conduct comprehensive in-home assessments for new AFC applicants.
  • Evaluate medical, functional, and psychosocial conditions to determine eligibility.
  • Partner with caregivers, families, and healthcare professionals to design customized care plans.
  • Provide education to participants and families regarding AFC services, expectations, and program policies.
  • Ensure compliance with all state regulations and AFC program requirements.
  • Maintain accurate, up-to-date documentation of assessments, care plans, and follow-up activities.

Qualification:

• Demonstrated ability to coach, train and provide staff development

• Proficiency in Microsoft (Word, Excel and PowerPoint)

• MS or Bachelor's degree in Nursing

• Must be fully licensed RN by Mass Board of Registration

• At least five years of previous home health care experience
