Salary: $106,000–$149,000 annually
Position OverviewWe are seeking a dedicated Nurse Practitioner or Physician Assistant to join an interdisciplinary healthcare team providing comprehensive primary care to older adults enrolled in a community‑based care program. This role focuses on preventative care, chronic disease management, acute care needs, and coordination of services across multiple care settings.
Key Responsibilities- Conduct comprehensive annual and semi‑annual physical examinations.
- Evaluate and treat patients in various settings, including the clinic, homes, assisted living facilities, and skilled nursing facilities.
- Provide episodic care, management of acute illnesses, and post‑hospitalization follow‑up.
- Perform home visits for homebound patients as clinically indicated.
- Conduct nursing facility visits in accordance with regulatory requirements and patient needs.
- Participate in an after‑hours on‑call rotation.
- Collaborate with physicians and interdisciplinary team members to develop and implement individualized care plans.
- Educate patients, families, caregivers, and team members regarding health conditions, treatment plans, and preventative care.
- Maintain accurate, timely, and compliant clinical documentation.
- Support high‑quality, patient‑centered care and positive health outcomes.
- Travel locally between care sites as needed.
- Perform additional duties as assigned.
- Master of Science in Nursing (MSN) or completion of an accredited Physician Assistant program.
- Current Board Certification as a Nurse Practitioner or Physician Assistant.
- Active Massachusetts NP or PA license.
- Current DEA registration and prescriptive authority in Massachusetts.
- Current CPR certification.
- Minimum of 2 years of experience working with a geriatric population (preferred).
- Strong clinical assessment, diagnostic, and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Strong organizational skills and attention to detail.
- Self‑motivated with effective time‑management skills.
- Sound clinical judgment and problem‑solving abilities.
- Ability to recognize and address clinical concerns proactively.
- Experience in geriatrics, long‑term care, PACE, primary care, home‑based care, or community health settings.
- Mandarin language proficiency (preferred).
- Participation in after‑hours on‑call coverage.
- Frequent local travel required.
- Ability to pass a respirator fit test; position may require use of a fitted mask where a proper seal is necessary.
- COVID‑19 vaccination (preferred).
- Annual influenza vaccination required.
