About This Role

The RN Case Review Coordinator is responsible for performing essential case management activities to facilitate safe and cost-effective transitions post-discharge. This role is ideal for experienced nurses with a background in utilization review and case management.

• Perform utilization review to evaluate appropriate level of care.
• Collaborate with patients, families, and interdisciplinary teams for discharge needs.
• Implement referrals to post-acute care.
• Document interactions with patients and teams online.
• Interact with third‑party payers for clinical communications.
• Refer cases not meeting care levels to the Physician Advisor.
• Review and facilitate documentation for Medicare status changes.
• Monitor quality issues and document in R.L. solutions.

Education:

• BSN required.

Experience:

• 3 years of Utilization Review and Case Management experience required.
• 5 years of Acute Care Nursing required.

Licenses &

Certifications:

• Current Registration as a Licensed Nurse in the State of Massachusetts.
