Home Health Nurse; RN

• Visit patients at home to assess health, safety, and daily living needs
• Determine eligible services, such as home health aides or therapy programs
• Coordinate care plans with healthcare providers and community organizations
• Communicate effectively with families regarding care plans and requirements
• Maintain detailed records of assessments and interventions

• Valid RN license in Massachusetts
• Associate degree or higher in Nursing

• Minimum of 1 year of nursing experience
• Community nursing experience preferred
• Interest in working with elderly populations
• Strong organization, communication, and clinical judgment skills
• Ability to work independently and coordinate care effectively
